General terms and conditions of use Health Premium Card
IDCQ Hospitales y Sanidad, S.L.U., as owner of the Hospital Quirónsalud Torrevieja, with address for the purposes of this document at Partida de la Loma, s/n, 03180, Torrevieja, Alicante and NIF B-87324844 (hereinafter, "Quirónsalud") offers the possibility of obtaining the HEALTH PREMIUM CARD through which its holders can benefit from exclusive discounts and services.
CONDITIONS:
1. HEALTH PREMIUM CARD grants its holders the right to benefit from the discounts and privileges listed at www.healthpremiumcard.com and to receive preferential services also listed in this link. The HEALTH PREMIUM CARD card only entitles the holder to benefits and care only at the Hospital Quirónsalud Torrevieja (Partida de la Loma, s/n, 03180, Torrevieja, Alicante).
2. The HEALTH PREMIUM CARD loyalty card is governed by the rules and conditions of use set out in these General Conditions. The HEALTH PREMIUM CARD implies full acceptance of the same by its holder.
3. To acquire the HEALTH PREMIUM CARD, the user must fill in the Card Application form with his/her details and the SEPA form and accept these General Conditions. The HEALTH PREMIUM CARD is delivered on the same day that the HEALTH PREMIUM CARD is paid for.
4. The HEALTH PREMIUM CARD is individual (the name of the cardholder appears on the card), personal and non-transferable. In order to enjoy the benefits and preferential care associated with it, the cardholder must always present his/her identification document (DNI, NIE or passport) together with his/her card.
5. The card does not confer any financial benefit on the holder for the purchase of medicines, devices or other health and medical prescription needs.
6. Quirónsalud will notify the cardholder of any important change in these General Conditions and/or in the services and benefits indicated on the HEALTH PREMIUM CARD. In any case, these changes will only be applicable from the date of renewal of the PLAN in accordance with point 8 below, so that the cardholder may object to the renewal of the same sufficiently in advance.
7. The cost of the HEALTH PREMIUM CARD is THREE HUNDRED EUROS (300€) per year. A single payment will be made in advance by bank card.
8. The HEALTH PREMIUM CARD is valid for one (1) year from the date of payment. Once the period of validity has expired and the new annual fee has been paid, the HEALTH PREMIUM CARD will be automatically renewed for another year.
In the event that the holder is not interested in renewing it, he/she must notify this by e-mail to healthpremium.trv@quironsalud.es
at least thirty (30) days prior to the expiry date of its validity.
9. The cardholder undertakes to make good use of the card at all times and to comply fully with these General Conditions. Quirónsalud reserves the right to cancel the card if it considers that it is being misused. In such cases, Quirónsalud will return to the cardholder the proportional part of the term paid up to the end of the annual period.
10. The holder is obliged to inform Quirónsalud of any modification to their personal data. To do so and to make any enquiries, please write to the following email address: healthpremium.trv@quironsalud.es
11. PERSONAL DATA - Data Controller: IDCQ HOSPITALES Y SANIDAD, S.L.U. with registered office at Calle Zurbarán 28, Madrid (28010). DPO contact details: dpo@quironsalud.es. Data processed: identification and financial data. Origin: the data subject himself/herself. Purpose of processing: to manage your subscription to the HEALTH PREMIUM CARD. Legitimation: execution of a contract to which the data subject is a party. Retention: they will be kept for the duration of the contractual relationship, as well as for the period necessary for the formulation, exercise or defence of claims, at least five years. Recipients: Your personal data will not be disclosed to third parties unless legally obliged to do so. Rights: You may exercise your rights of access, rectification, deletion, opposition, portability and limitation of processing to: IDCQ HOSPITALES Y SANIDAD, S.L.U., at the following address: calle Zurbarán 28, Madrid (28010), with the reference "data protection rights", providing documentation accrediting your identity and indicating the right you wish to exercise. We also inform you of the possibility of lodging a complaint with the competent supervisory authority, in accordance with the procedure that corresponds to the specific case. 12. In the event of any doubt arising regarding the interpretation or fulfilment of this Contract, the parties, waiving any other jurisdiction that may correspond to them, submit to the exclusive jurisdiction of the Courts and Tribunals of Torrevieja (Alicante).
